Most people who die following a nuclear exchange won't die from the bomb itself but from the effects the bomb has on society. No more electricity, no modern medicine, very little food, likely NO law enforcement leading to wide spread violence. The nukes will only be the trigger mechanism for things that are far worse and far deadlier.
